All information provided on the application form is self declared except for proof of address, and a picture ID.

Currently, the mobile pantry delivers to North Pointe and Meadowridge apartment complexes on the second Tuesday of each month and to Lakota Pointe Towne Homes the third Thursday of each month from 5:00pm to 6:00pm. We park the truck at the Managers office in North Pointe at 5215 Westwind Avenue ; and the parking lot by the manager's office at Lakota Pointe Towne Homes. The participants meet our truck to pick up their food.

The first time they pick up their food, they must sign an "Eligibility to Take Food Home Form" (we provide) and have produce a picture identification. After their first pick up, they just sign a log sheet showing receipt of the food after showing their picture ID. We will assist in delivering directly to their apartments when necessary.

If a participant misses their pick up for two consecutive months, we will send a letter informing them they have been removed from the program.

Application Process
Completed applications should be faxed to Tina (513.874.0152) no later than the 25th of each month in order for the participant to be approved and notified of their eligibility to pick up food the following month. Applications are valid for one year, with renewals in July.
